Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations, including OASIS assessments as required.
• Develop and implement customized care plans addressing strength, mobility, balance, gait, pain management, and safety.
• Deliver skilled interventions: therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention.
• Assess progress, update plans of care, and communicate changes to all care team members.
• Provide education for patients and caregivers on home exercise routines, safety awareness, and equipment.
• Complete accurate, timely documentation in Home Care Home Base (HCHB).
• Collaborate closely with interdisciplinary team members to ensure holistic care.
• Participate in interdisciplinary meetings and adhere to all compliance standards.
• Travel daily within assigned territory (mileage reimbursement provided).

Qualifications:

• Current, active Physical Therapist license in good standing.
• At least one year of recent home health PT experience required.
• OASIS assessment experience required.
• Proficiency with Home Care Home Base (HCHB) preferred.
• Strong clinical assessment, time management, and communication skills.
• Must have reliable transportation, valid drivers license, and auto insurance.
