Deliver meaningful care and make a real difference as a dedicated Home Health Physical Therapist, serving patients in a community setting. This contract role offers the opportunity to help individuals regain movement, independence, and confidence in the comfort of their own homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Qualifications:

• Active Physical Therapist (PT) license in good standing
• Minimum of one year of recent experience providing Physical Therapy in the home health setting (fieldwork preferred)
• OASIS assessment experience is required
• Proficiency with Home Care Home Base (HCHB) is preferred
• Strong clinical assessment and decision-making abilities
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently, managing your time and caseload efficiently
• Exceptional communication, organization, and patient education skills

Key Responsibilities:

• Complete comprehensive evaluations, including OASIS assessments for new admissions, resumptions, recertifications, and discharges
• Develop and implement individualized therapy plans focused on improving mobility, balance, strength, safety, and function
• Deliver skilled interventions such as therapeutic exercise, transfer training, gait training, neuromuscular re-education, and fall prevention
• Educate patients and caregivers on care plans, exercises, safety measures, and proper use of assistive devices
• Accurately document patient encounters and progress in Home Care Home Base (HCHB)
• Communicate regularly with physicians and interdisciplinary team members to ensure optimal patient outcomes
• Participate in case conferences, interdisciplinary meetings, and continuing education as needed
