Job Description

Experienced Physical Therapists with a passion for home health will find a rewarding opportunity delivering care in a dynamic, patient-centered field setting. This contract position focuses on providing skilled therapy for patients in their homes, with the goal of improving mobility, strength, and independence. You will work closely with an interdisciplinary care team while independently managing your caseload.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract

Required Qualifications:

  • Active Physical Therapist (PT) license in the state of practice, in good standing
  • At least 1 year of recent Home Health Physical Therapy experience
  • OASIS assessment experience required
  • Proficient or familiar with electronic documentation systems (Home Care Home Base preferred)
  • Skilled at performing comprehensive functional assessments and clinical decision-making
  • Reliable transportation, valid drivers license, and current auto insurance
  • Excellent organization and communication skills, with the ability to manage a flexible, field-based workload

Key Responsibilities:

  • Conduct thorough physical therapy evaluations and OASIS assessments as needed
  • Create and implement personalized care plans to address individual patient goals
  • Deliver evidence-based interventions, including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
  • Track progress, adapt treatment, and communicate with physicians and the care team
  • Provide patient and caregiver education on safety, home exercise, and device usage
  • Maintain accurate electronic documentation in line with agency and Medicare standards
  • Collaborate with nursing, occupational, and speech-language therapy, as well as medical social work and home health aides
  • Participate in conferences, interdisciplinary meetings, and ongoing professional development
