Job Description

An experienced Home Health Physical Therapist (PT) is needed to provide comprehensive, patient-centered therapy services in a home-based setting. This role focuses on enhancing patient mobility, strength, balance, and overall functional independence through skilled evaluations and interventions tailored to individual needs. The position offers a dynamic work environment with autonomy, making it ideal for self-motivated and organized professionals comfortable working independently while delivering quality care.

Key Responsibilities:

  • Perform thorough physical therapy evaluations, including OASIS assessments when applicable.
  • Develop and execute individualized treatment plans targeting functional mobility, strength, balance, gait, pain management, and safety.
  • Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
  • Monitor patient progress, update care plans as needed, and communicate patient status to physicians and interdisciplinary teams.
  • Educate patients and caregivers on exercise techniques, safety measures, assistive device use, and home exercise regimens.
  • Maintain timely, accurate documentation using Home Care Home Base (HCHB) electronic system.
  • Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides to ensure coordinated care.
  • Participate in case conferences, team meetings, and ongoing professional development.
  • Adhere strictly to agency policies, regulatory requirements, and Medicare guidelines.

Qualifications:

  • Active Physical Therapist license in good standing within the state of practice.
  • Minimum of one year recent home health physical therapy experience, with field experience preferred.
  • Proficiency in Home Care Home Base (HCHB) and OASIS assessment experience required.
  • Strong clinical decision-making and functional assessment expertise.
  • Reliable transportation, valid drivers license, and automobile insurance.
  • Ability to work independently and manage a flexible field-based schedule.
  • Excellent communication, time management, and organizational skills.

Location: This position is based in Stone Mountain, Georgia, a vibrant community rich in natural beauty and historic sites. The area offers easy access to outdoor recreational activities and is part of a larger metropolitan region providing cultural attractions, shopping, and dining options.

Contract Type & Schedule:

  • Full-time, 13-week contract.
  • Monday through Friday, 8:00 AM to 5:00 PM.
  • Caseload includes 2532 points weekly, with approximately 57 patient visits daily.
  • Daily travel to patient homes with mileage reimbursement provided.
