An opportunity is available for an experienced Home Health Physical Therapist (PT) who is skilled in delivering patient-centered care in a field-based setting. This full-time, 13-week contract position involves providing physical therapy services directly to patients in their homes within the Soperton, GA area. The role requires a compassionate, self-motivated therapist comfortable working independently while collaborating remotely with an interdisciplinary healthcare team.

Responsibilities include conducting comprehensive physical therapy evaluations, including OASIS assessments; designing and implementing individualized treatment plans focused on improving mobility, strength, balance, gait, pain management, and safety; delivering skilled interventions such as therapeutic exercise and gait training; educating patients and caregivers; and maintaining accurate documentation using Home Care Home Base (HCHB). Continuous communication with physicians and healthcare colleagues is essential to ensure coordinated and effective care.

Key qualifications include an active Physical Therapist license in good standing, a minimum of one year recent home health PT experience, proficiency or familiarity with the HCHB electronic documentation system, OASIS assessment experience, and reliable transportation. Strong clinical judgment, excellent communication skills, and the ability to manage time efficiently are crucial for success in this independent practice setting.

Schedule & Workload:

• Monday through Friday, 8:00 AM 5:00 PM
• Caseload approximately 2532 points weekly (57 patient visits daily)
• Daily travel within assigned territory with mileage reimbursement provided
