Deliver essential physical therapy services in a rewarding home health environment, providing care that directly supports patients independence and quality of life. This field role involves traveling to patients residences within an assigned territory, conducting comprehensive therapy evaluations, and developing tailored treatment plans. The position is structured as a full-time, 13-week contract, offering a consistent Monday through Friday, 8:00 AM to 5:00 PM schedule.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete physical therapy evaluations at patient homes, utilizing OASIS for necessary assessments.
• Implement customized treatment plans enhancing mobility, balance, strength, gait, and safe function.
• Deliver interventions such as gait training, therapeutic exercises, transfer training, and fall prevention strategies.
• Educate patients and caregivers on mobility, exercises, safety, and the use of assistive devices.
• Maintain timely and accurate documentation using Home Care Home Base (HCHB).
• Collaborate with other clinicians (RN, OT, SLP, MSW, aides) to ensure coordinated, patient-centered care.
• Attend interdisciplinary meetings and participate in continuing education.
• Comply with all regulatory standards and Medicare guidelines while adhering to agency policies.

Qualifications:

• Active Physical Therapist (PT) license in good standing within the state.
• Minimum 1 year of recent home health field PT experience.
• OASIS assessment proficiency required.
• Experience with Home Care Home Base (HCHB) preferred.
• Effective clinical judgment, communication, and organizational skills.
• Reliable vehicle, valid drivers license, and current auto insurance.
• Ability to independently manage a field-based caseload and flexible schedule.
