Make a meaningful impact by delivering high-quality, one-on-one rehabilitation services to patients in their homes as a Home Health Physical Therapist. This contract opportunity is designed for skilled professionals who are passionate about helping individuals restore mobility, independence, and quality of life while working independently in a supportive, interdisciplinary environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

What You'll Do:

• Complete comprehensive assessments, including OASIS evaluations as required.
• Develop and execute individualized care plans that target patient mobility, strength, balance, gait, and overall function.
• Implement evidence-based interventions, including therapeutic exercises, gait and transfer training, neuromuscular re-education, and fall prevention strategies.
• Evaluate patient progress and update treatment plans in collaboration with healthcare team members.
• Educate patients and caregivers on exercises, home safety, the use of assistive devices, and self-management strategies.
• Document all care activities and progress using Home Care Home Base (HCHB).
• Travel daily to patient residences within the assigned territory (mileage reimbursement provided).
• Actively participate in interdisciplinary case conferences and meetings.

What Were Looking For:

• Current, unrestricted Physical Therapist license in the state of practice
• Minimum 1 year of recent home health physical therapy field experience
• Proficiency with Home Care Home Base (HCHB) strongly preferred
• OASIS assessment experience required
• Excellent organizational, time management, and clinical decision-making abilities
• Strong written and verbal communication skills
• Reliable transportation, valid drivers license, and proof of auto insurance
