Deliver high-quality care where it matters most as a Home Health Physical Therapist. You'll focus on restoring mobility, increasing strength and balance, and empowering individuals to achieve greater independence in a supportive environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Qualifications:

• Active and current Physical Therapist (PT) license in the state of practice
• At least one year of recent home health physical therapy experience, with field-based experience strongly preferred
• Prior experience using Home Care Home Base (HCHB) preferred
• Proficiency with OASIS assessments required
• Strong clinical judgment, assessment, and care planning capabilities
• Reliable transportation along with a valid drivers license and auto insurance
• Ability to independently manage a field-based schedule and caseload
• Outstanding communication, organization, and time management skills

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments
• Create and implement tailored treatment plans addressing strength, mobility, gait, pain management, and safety
• Deliver skilled therapeutic interventions including exercise programs, neuromuscular re-education, and gait training
• Monitor progress, adapt care plans as necessary, and communicate updates to physicians and the interdisciplinary care team
• Educate patients and caregivers on exercises, fall prevention, and safe use of assistive devices
• Maintain clear and timely documentation in Home Care Home Base (HCHB)
• Participate in interdisciplinary meetings and adhere to regulatory and practice standards
