Job Description
Deliver essential rehabilitative care as a Home Health Physical Therapist, providing skilled therapy services to patients in their homes. This contract position is ideal for a self-driven healthcare professional dedicated to maximizing patient mobility and independence within a supportive, collaborative environment.
Schedule:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete thorough physical therapy evaluations, including OASIS assessments as needed.
- Develop and implement tailored plans of care focused on improving mobility, strength, balance, gait, and functional safety.
- Administer skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, pain management, and fall prevention.
- Assess patient outcomes, adjust treatment plans accordingly, and consistently communicate with the interdisciplinary home health team.
- Educate patients and caregivers on home exercise programs, safety, and proper use of assistive devices.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB).
- Regularly collaborate with nursing staff, therapy disciplines, social workers, and home health aides.
- Attend case conferences and interdisciplinary meetings as necessary while adhering to agency standards and Medicare home health regulations.
Desired Skills & Experience:
- Active Physical Therapist (PT) license in the state of practice
- At least one year of recent field-based Home Health PT experience
- Experience using Home Care Home Base (HCHB) preferred
- Required proficiency with OASIS documentation
- Reliable transportation, valid drivers license, and current auto insurance
- Strong clinical judgment, organizational, and communication skills
