An exciting opportunity awaits for a skilled and compassionate physical therapist to provide essential home health services. As a Home Health PT, you will deliver rehabilitative care directly in patients homesfostering improved mobility, strength, and overall independence for individuals in need of personalized therapy. This role is ideal for a clinician who is self-motivated, organized, and experienced with home health care protocols.

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Expectation of approximately 2532 points per week (typically 57 patient visits per day, based on visit type)

Desired Qualifications:

• Active state Physical Therapist (PT) license in good standing
• At least 1 year of recent home health field experience
• Proficiency with Home Care Home Base (HCHB) preferred
• Required: OASIS assessment experience
• Strong clinical judgment and experience with functional assessments
• Reliable transportation, valid drivers license, and insurance
• Able to work independently, manage a flexible schedule, and communicate effectively across disciplines

Responsibilities Include:

• Perform comprehensive physical therapy evaluations (including OASIS at Start of Care, Resumption, Recertification, and Discharge as needed)
• Develop and implement individualized treatment plans focused on improving function, safety, and independence
• Deliver skilled interventions: therapeutic exercise, neuromuscular re-education, gait training, transfer training, and fall prevention
• Track progress and adapt care in collaboration with physicians and interdisciplinary team members
• Provide education to patients and caregivers on exercise and safety strategies
• Ensure timely and thorough documentation using HCHB
• Attend case conferences and contribute to coordinated patient care

$1,718 - $1,946/Week Estimated weekly earnings represent hourly wages, based on 32 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
