Job Description

Make a meaningful impact on patients lives by delivering rehabilitative care directly in their homes. A skilled and compassionate clinician is needed to provide physical therapy services, assess mobility and functional status, and create tailored treatment plans for clients in a home health setting. This contract, full-time role spans 13 weeks and offers a consistent weekday schedule.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Core Qualifications:

  • Active Physical Therapist license in good standing within the state
  • At least one year of recent experience in home health physical therapy
  • Proficient with OASIS documentation
  • Familiarity with Home Care Home Base (HCHB) preferred
  • Solid clinical skills in assessment and treatment planning
  • Reliable transportation, valid drivers license, and appropriate insurance
  • Strong organizational, communication, and time management abilities
  • Comfortable working independently in a field-based role

Primary Responsibilities:

  • Perform thorough physical therapy evaluations, including OASIS assessments for new admissions and follow-ups
  • Collaborate with interdisciplinary team members to coordinate patient care and communicate progress
  • Develop and update individualized care plans to address patient mobility, balance, gait, pain, and safety needs
  • Provide interventions such as exercise programs, gait and transfer training, fall prevention, and caregiver education
  • Accurately document interventions and patient progress using electronic systems
  • Drive daily to patient homes within the assigned area, with mileage reimbursement provided
  • Participate in required meetings, continuing education, and maintain compliance with all agency and regulatory standards
