Provide skilled, patient-centered physical therapy services in home health settings with this full-time, 13-week contract opportunity located in Silver Spring, MD. This role involves delivering comprehensive evaluations and individualized treatment plans to promote mobility, strength, balance, and independence for patients in their homes. The schedule is Monday through Friday, 8:00 AM to 5:00 PM, with a typical caseload of 2532 points weekly (approximately 57 visits per day).

Responsibilities:

• Perform thorough physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge.
• Develop and implement individualized plans of care targeting functional mobility, gait, pain management, balance, and safety.
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
• Monitor patient progress, adjust treatment plans accordingly, and communicate updates to physicians and healthcare team members.
• Educate patients and caregivers on exercises, safety procedures, home exercise programs, and assistive device use.
• Maintain accurate documentation in Home Care Home Base (HCHB).
• Collaborate with interdisciplinary healthcare professionals to ensure coordinated, high-quality care.
• Participate in case conferences and ongoing education.
• Comply with agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

• Current Physical Therapist (PT) license in good standing within the state.
• Minimum 1 year of recent home health physical therapy experience; field-based experience preferred.
• Proficient with Home Care Home Base (HCHB) and experienced with OASIS assessments.
• Strong clinical judgment and functional assessment abilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently with excellent communication, time management, and organizational skills.
