Deliver essential care directly to patients homes as a Home Health Physical Therapist. This rewarding contract opportunity focuses on empowering individuals in a home-based setting, supporting their journey toward improved mobility, strength, and independence. The position is ideal for a self-driven, detail-oriented clinician who thrives with autonomy and values patient-centered outcomes.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Approximate caseload: 2532 points per week (typically 57 visits per day)
Key Responsibilities:
- Perform comprehensive physical therapy assessments, including required OASIS documentation, within patient homes.
- Develop and implement tailored care plans, addressing functional mobility, pain, balance, gait, strength, and patient safety.
- Deliver skilled interventions, such as therapeutic exercise, gait training, transfer techniques, neuromuscular re-education, and fall prevention.
- Educate patients and caregivers on exercise routines, assistive device use, and safe mobility strategies.
- Maintain up-to-date, detailed clinical documentation using Home Care Home Base (HCHB).
- Reassess patient progress and update treatment plans in line with patient status and physician guidance.
- Collaborate with RNs, occupational therapists, SLPs, social workers, and other team members for coordinated care.
- Comply with all home health regulatory requirements and agency standards.
- Participate in multidisciplinary case conferences and professional education.
Qualifications:
- Active Physical Therapist (PT) license (in good standing)
- Minimum 1 year of recent home health PT experience
- OASIS assessment experience required
- Proficient in Home Care Home Base (HCHB) preferred
- Strong clinical judgment with excellent communication and organization
- Reliable transportation, valid drivers license, and auto insurance
