Experienced and compassionate Physical Therapists passionate about delivering skilled home health care are needed to provide high-quality, patient-centered services. This 13-week contract role offers the chance to work one-on-one with patients in their homes, helping them achieve optimal mobility and independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete thorough physical therapy evaluations, including OASIS assessments when required
• Develop and execute individualized plans of care emphasizing mobility, strength, gait, balance, and pain management
• Provide skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
• Monitor patient progress, revise treatment plans as needed, and proactively communicate with physicians and interdisciplinary team members
• Educate patients and caregivers about exercises, use of assistive devices, safety strategies, and home programs
• Maintain timely, accurate documentation using Home Care Home Base (HCHB)
• Collaborate with nurses, occupational and speech therapists, social workers, and home health aides for coordinated patient care
• Participate in case conferences and interdisciplinary meetings as required
• Adhere to all relevant agency, state, and Medicare regulatory requirements

Qualifications:

• Current, active Physical Therapist (PT) license in good standing for the state of practice
• Minimum 1 year of recent field-based home health physical therapy experience required
• OASIS documentation proficiency required; experience using Home Care Home Base (HCHB) highly preferred
• Strong assessment, critical thinking, and communication skills
• Reliable transportation, a valid drivers license, and current auto insurance
• Ability to work autonomously and manage a flexible schedule

$2,839 - $3,003/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
