Delivering vital rehabilitative care directly to patients in their homes, this contract opportunity is ideal for a skilled and compassionate Physical Therapist with home health experience. Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Qualifications:

• Active and current Physical Therapist (PT) license in good standing
• At least 1 year of recent Home Health PT experience, with direct field experience preferred
• Proficiency with Home Care Home Base (HCHB) software preferred
• Demonstrated expertise performing OASIS assessments
• Strong clinical judgment and functional assessment skills
• Reliable transportation with valid drivers license and auto insurance
• Ability to work autonomously and adapt to a flexible, field-oriented schedule
• Exceptional organizational, time management, and communication skills

Primary Responsibilities:

• Conduct thorough physical therapy evaluations, including all required OASIS assessments
• Design and implement individualized treatment plans to foster improvements in strength, mobility, balance, and safety
• Deliver skilled interventions such as therapeutic exercise, gait training, transfer training, neuromuscular re-education, and fall prevention
• Monitor patient progress, adjusting plans of care and communicating updates to physicians and team members
• Educate patients and families on exercises, safety protocols, and the use of assistive devices
• Maintain timely, accurate documentation in HCHB
• Collaborate within an interdisciplinary team for holistic patient care
• Participate in clinical case conferences and ongoing educational activities
• Ensure compliance with agency policies and applicable healthcare regulations

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
