Experienced physical therapists passionate about patient-centered care are invited to deliver vital services in a rewarding, field-based role. This position focuses on supporting patients as they regain independence and mobility in the comfort of their own homes.
Schedule:
- Full-time, contract position (13 weeks)
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (typically 57 home visits per day)
Role Overview: You will conduct comprehensive evaluations, develop and implement individualized physical therapy care plans, and collaborate closely with interdisciplinary healthcare professionals to promote the best outcomes. Emphasis is on functional mobility, balance, patient education, and empowering patients and caregivers for long-term success.
Key Responsibilities:
- Complete thorough patient assessments, including required OASIS evaluations
- Develop and adjust tailored treatment plans for strengthening, balance, pain management, gait, safe transfers, and fall prevention
- Deliver skilled interventions through evidence-based therapeutic techniques
- Educate patients and caregivers on exercise regimens, safety protocols, use of assistive devices, and at-home wellness
- Maintain up-to-date, accurate clinical documentation using Home Care Home Base (HCHB)
- Participate in case conferences and interdisciplinary meetings
- Ensure adherence to Medicare, regulatory, and agency standards
- Travel daily within your assigned territory, offering hands-on support for each patient
Qualifications:
- Active and unencumbered Physical Therapist (PT) license in Washington
- Minimum one year of recent home health PT experience required
- Skilled in Home Care Home Base (HCHB) and OASIS documentation
- Strong assessment, communication, and organizational abilities
- Reliable transportation, valid drivers license, and auto insurance
- Comfortable working independently with a flexible schedule
