Deliver high-quality, patient-focused care as a Home Health Physical Therapist caring for individuals in their homes. This contract role enables you to make a meaningful difference while utilizing your expertise in rehabilitation and patient education to improve mobility, strength, and overall independence for each patient.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based: daily travel to patient homes within your assigned territory (mileage reimbursement provided)
• Caseload: approximately 2532 points per week (57 visits per day, depending on visit type)

What Youll Do:

• Perform thorough physical therapy evaluations, including OASIS assessments as required for start of care, resumption, recertification, and discharge
• Develop, execute, and modify individualized care plans that address functional mobility, fall risks, pain management, and safety
• Administer skilled interventions such as therapeutic exercises, gait and balance training, neuromuscular re-education, and transfer training
• Educate patients and caregivers on safe practices, assistive devices, and tailored home exercise programs
• Accurately document all care activities in Home Care Home Base (HCHB)
• Communicate patient progress, needs, and changes promptly with physicians and the multidisciplinary team
• Participate in collaborative care planning and regular case conferencing

Qualifications:

• Active and unrestricted Physical Therapist license for Pennsylvania
• At least 1 year of recent home health PT experience (field-based strongly preferred)
• Proficiency in Home Care Home Base (HCHB) preferred
• OASIS experience required
• Effective communication, organizational, and assessment skills
• Reliable transportation, a valid drivers license, and active auto insurance

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
