A dedicated opportunity awaits for an experienced, compassionate clinician to provide vital care as a Home Health Physical Therapist. This contract role focuses on delivering high-quality, patient-centered rehabilitation to clients in their homes. You'll help individuals regain strength, mobility, balance, and independence, supporting their recovery through skilled therapy and education.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• 13-week contract
• Field-based, with approximately 2532 points per week (57 patient visits daily, dependent on visit types)

Essential Qualifications:

• Active, unrestricted Physical Therapist (PT) license in the state
• At least 1 year of recent Home Health PT experience
• OASIS assessment experience required
• Familiarity with Home Care Home Base (HCHB) software preferred
• Proficient clinical assessment and decision-making skills
• Reliable transportation, current driver's license, and auto insurance
• Ability to work independently and manage a flexible field schedule
• Excellent communication, time management, and organizational abilities

Core Responsibilities:

• Perform comprehensive evaluations, including OASIS assessments
• Create and adapt individualized treatment plans targeting function, gait, balance, pain, and mobility
• Deliver skilled interventions: therapeutic exercise, neuromuscular re-education, gait/transfer training, fall prevention
• Educate patients and caregivers on home exercise programs, equipment, and safety strategies
• Track patient progress and update care plans accordingly
• Collaborate within an interdisciplinary team for integrated care
• Accurately document all care in HCHB and adhere to agency, regulatory, and Medicare guidelines
• Participate in case conferences and continuing education

$2,727 - $2,885/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
