• Educate patients and their families regarding exercise techniques, safe transfers, and proper use of assistive devices
• Provide ongoing therapy interventions to help patients regain strength and improve functional independence

Qualifications and Desired Experience:

• Degree in Physical Therapy from an accredited institution
• Current and valid state Physical Therapy license
• Experience in Home Health and Home Care with HCHB preferred
• Valid drivers license and reliable transportation to travel to patient homes
