An excellent opportunity is available for an experienced and compassionate Home Health Physical Therapist (PT) to provide high-quality, patient-centered care. In this contract position, you will apply your clinical skills to assist individuals in restoring mobility, improving strength and balance, and achieving greater independenceall within the comfort of their homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations, utilizing OASIS assessment tools.
- Develop and implement tailored care plans addressing mobility, strength, balance, gait, pain, and safety.
- Administer skilled interventions, such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
- Monitor patient progress, modify care plans as needed, and communicate changes with physicians and care teams.
- Educate patients and caregivers on therapeutic exercises, home safety, assistive devices, and home programs.
- Accurately and promptly document all care activities in Home Care Home Base (HCHB).
- Collaborate with interdisciplinary healthcare professionals to ensure continuity of care.
- Participate in case conferences, team meetings, and ongoing professional development.
- Adhere to agency policies, regulatory requirements, and Medicare guidelines.
Qualifications:
- Active PT license in good standing.
- Minimum of 1 year recent experience in home health physical therapy.
- Proficiency with Home Care Home Base (HCHB) preferred; OASIS experience required.
- Strong decision-making, clinical assessment, and communication skills.
- Reliable transportation, valid drivers license, and current auto insurance.
- Capable of managing an independent, field-based workload.
