An excellent opportunity awaits for a dedicated Home Health Physical Therapist with strong clinical expertise in field-based care. This role focuses on providing skilled home health physical therapy to individuals seeking to regain or enhance their mobility, strength, and functional independence. This position is offered as a full-time, 13-week contract.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based role: daily travel required to visit patients in their homes
• Caseload: approximately 2532 points per week (generally 57 patient visits per day)
• Documentation through Home Care Home Base (HCHB)
• OASIS assessments as required

Desired Qualifications:

• Current, active Physical Therapist (PT) license in good standing
• Minimum one year of recent home health PT experience; field experience strongly preferred
• Proficient with Home Care Home Base (HCHB) electronic medical records preferred
• Previous completion of OASIS evaluations (Start of Care, Resumption, Recertification, Discharge)
• Solid clinical assessment skills and adaptable clinical judgment
• Reliable transportation, valid drivers license, auto insurance
• Self-motivated, organized, independent work style
• Clear, professional communication and time management

Key Responsibilities:

• Perform comprehensive patient evaluations, including OASIS where applicable
• Create, implement, and adapt individualized care plans focusing on mobility, gait, balance, strength, and pain management
• Deliver skilled physical therapy interventions such as therapeutic exercise, gait and transfer training, and fall prevention
• Educate clients and caregivers on therapeutic exercises, assistive devices, and safety
• Maintain timely and accurate documentation in Home Care Home Base
• Collaborate closely with allied healthcare professionals for optimal patient outcomes
• Participate in interdisciplinary case conferences and ongoing education
• Comply with agency, regulatory, and Medicare guidelines

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
