Experienced and compassionate physical therapists play a critical role in home health care, making a measurable difference in patient outcomes and independence. This contract opportunity offers you the chance to deliver skilled physical therapy services directly in patients homes, empowering individuals to regain function, mobility, and quality of life.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points per week (57 patient visits daily)
- Field-based position with daily travel (mileage reimbursement provided)
Qualifications:
- Active Physical Therapist (PT) license in good standing
- Minimum 1 year of recent home health physical therapy experience (field experience strongly preferred)
- Proficiency using Home Care Home Base (HCHB) electronic documentation (preferred)
- OASIS assessment experience required
- Reliable transportation, valid drivers license, and auto insurance
- Strong communication, clinical decision-making, and organizational skills
- Ability to work independently and manage a flexible field-based schedule
Essential Responsibilities:
- Conduct comprehensive PT evaluations and OASIS assessments as required
- Develop and implement individualized care plans targeting mobility, strength, balance, gait, and safety
- Deliver skilled interventions (therapeutic exercise, transfer/gait training, neuromuscular re-education, fall prevention)
- Educate patients/caregivers on safety, home exercise, and use of assistive devices
- Maintain timely and accurate documentation in Home Care Home Base (HCHB)
- Collaborate with interdisciplinary healthcare team members
- Participate in case conferences and continuing education as required
- Ensure compliance with agency policies and Medicare/clinical standards
