Job Description
Provide expert physical therapy care as a Home Health Physical Therapist (PT) delivering skilled services to patients in their residences. This role requires a motivated professional capable of conducting thorough evaluations, developing personalized treatment plans, and collaborating with healthcare teams to enhance patient mobility, strength, balance, and overall functional independence.
Key Responsibilities:
- Perform comprehensive evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge when applicable
- Design and implement individualized treatment plans addressing mobility, pain management, gait training, and fall prevention
- Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, and transfer training
- Monitor patient progress, adjust treatment plans, and communicate changes with physicians and interdisciplinary team members
- Educate patients and caregivers on exercise regimens, safety strategies, assistive device use, and home exercise programs
- Maintain accurate, timely documentation using Home Care Home Base (HCHB) electronic system
- Participate in case conferences, interdisciplinary meetings, and adhere to Medicare home health guidelines and agency policies
Qualifications:
- Current Physical Therapist license in good standing for the state of practice
- Minimum one year of recent Home Health Physical Therapy experience; field experience strongly preferred
- Proficiency with OASIS assessment tool is required; familiarity with Home Care Home Base (HCHB) preferred
- Excellent clinical judgment, communication, organizational, and time management skills
- Reliable transportation, valid drivers license, and auto insurance to support daily travel within assigned territory
- Ability to work independently in a field-based environment
Schedule & Location:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
- Home-based care across the Sahuarita, Arizona area with mileage reimbursement provided
