You will be responsible for evaluating patients physical conditions and crafting personalized treatment plans to enhance their mobility and overall well-being.

Key Responsibilities:

• Perform thorough initial assessments to determine patients' physical abilities and limitations.
• Design and implement individualized treatment plans aligned with patient goals and physician directives.
• Provide therapeutic exercises, manual therapy, and modalities to improve strength, range of motion, and independence.
• Educate patients and caregivers on proper exercise techniques, transfers, and assistive device use.
• Track and document patient progress, adjusting treatment plans as needed.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive care.
• Maintain accurate electronic medical records using HomeCare HomeBase (HCHB) software in compliance with regulatory standards.
• Uphold ethical and professional standards consistent with state and federal regulations.

Qualifications and Experience:

• Bachelors or Masters degree in Physical Therapy from an accredited institution.
• Current Physical Therapist license or eligibility for licensure in the state.
• Prior experience in a home health environment.
• Proficiency with HomeCare HomeBase (HCHB) electronic medical records.
• Strong clinical assessment skills and critical thinking ability.
• Excellent communication skills and a compassionate, patient-focused approach.
• Knowledge of Medicare guidelines related to home health care.
• Valid drivers license and reliable transportation to travel to patient residences.
