Job Description

Make a meaningful impact in the community by delivering patient-centered care as a Home Health Physical Therapist. This full-time, 13-week contract lets you work directly with patients in their homes, focusing on promoting mobility, independence, and quality of life. Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Perform comprehensive physical therapy evaluations, including OASIS assessments when needed
  • Develop and implement individualized treatment plans targeting mobility, strength, gait, pain management, and safety
  • Deliver skilled interventions such as therapeutic exercises, gait and transfer training, and fall prevention strategies
  • Track patient progress, adjust therapeutic approaches, and communicate updates to the care team and physicians
  • Educate patients and caregivers on home exercises, equipment use, and safety best practices
  • Document care promptly and completely in Home Care Home Base (HCHB)
  • Coordinate with nurses, occupational and speech therapists, social workers, and home health aides
  • Participate in care conferences, interdisciplinary meetings, and ongoing professional development

Qualifications:

  • Active Physical Therapist license (state-specific, in good standing)
  • At least 1 year of recent home health physical therapy experience
  • OASIS assessment experience required
  • Preferred experience with Home Care Home Base (HCHB)
  • Excellent clinical judgment, assessment, and communication skills
  • Reliable transportation and valid drivers license
