Job Description
Make a meaningful impact in the community by delivering patient-centered care as a Home Health Physical Therapist. This full-time, 13-week contract lets you work directly with patients in their homes, focusing on promoting mobility, independence, and quality of life. Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments when needed
- Develop and implement individualized treatment plans targeting mobility, strength, gait, pain management, and safety
- Deliver skilled interventions such as therapeutic exercises, gait and transfer training, and fall prevention strategies
- Track patient progress, adjust therapeutic approaches, and communicate updates to the care team and physicians
- Educate patients and caregivers on home exercises, equipment use, and safety best practices
- Document care promptly and completely in Home Care Home Base (HCHB)
- Coordinate with nurses, occupational and speech therapists, social workers, and home health aides
- Participate in care conferences, interdisciplinary meetings, and ongoing professional development
Qualifications:
- Active Physical Therapist license (state-specific, in good standing)
- At least 1 year of recent home health physical therapy experience
- OASIS assessment experience required
- Preferred experience with Home Care Home Base (HCHB)
- Excellent clinical judgment, assessment, and communication skills
- Reliable transportation and valid drivers license
