Job Description
Make a meaningful impact in patients lives by delivering in-home rehabilitative care as a Home Health Physical Therapist on a 13-week contract. Youll utilize your clinical expertise and compassionate approach to help patients regain mobility and independence in the comfort of their own homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments as required
- Develop and adjust individualized care plans focused on improving balance, mobility, strength, gait, and overall function
- Deliver skilled interventionstherapeutic exercises, gait and transfer training, neuromuscular re-education, pain management, and fall prevention
- Provide education to patients and caregivers on safety strategies, assistive devices, and home exercise routines
- Maintain accurate, timely documentation in Home Care Home Base (HCHB)
- Communicate patient progress or changes in status to physicians and other care team members
- Collaborate closely with nurses, occupational and speech therapists, medical social workers, and aides
- Attend case conferences and participate in interdisciplinary meetings as necessary
- Follow all agency protocols and Medicare home health guidelines
Qualifications:
- Active Physical Therapist (PT) license in the state of practice; in good standing
- At least one year of recent home health physical therapy experience
- Experience with OASIS assessments required
- Proficiency in Home Care Home Base (HCHB) preferred
- Strong clinical assessment, communication, and time management skills
- Reliable transportation, valid drivers license, and current auto insurance
- Comfort working autonomously while managing a variable, field-based caseload
