Deliver high-quality therapy services and empower patients to regain their independence as a Home Health Physical Therapist. This rewarding contract opportunity involves working directly with patients in their homes, helping them optimize their functional mobility, safety, and quality of life.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Qualifications:
- Active Physical Therapist (PT) license in the state of practice and in good standing
- Minimum of 1 year of recent home health physical therapy experience (field-based strongly preferred)
- Proficient in OASIS assessments and electronic documentation; Home Care Home Base (HCHB) experience preferred
- Reliable transportation, valid drivers license, and current auto insurance
- Strong clinical knowledge, independent decision-making, and excellent organizational and communication abilities
Core Responsibilities:
- Complete comprehensive physical therapy evaluations, including required OASIS assessments
- Develop, implement, and regularly update individualized care plans focused on mobility, strength, balance, pain management, and safety
- Deliver skilled interventions such as therapeutic exercise, gait, and transfer training, and fall prevention strategies
- Educate patients and caregivers on exercise routines, use of assistive devices, and safety in the home
- Document all care provided in the HCHB electronic system and ensure compliance with Medicare and regulatory guidelines
- Liaise with the interdisciplinary teamincluding nurses, therapists, social workers, and aidesto coordinate and support optimal patient outcomes
- Participate in team conferences, meetings, and continuing education sessions as required
