Deliver skilled physical therapy services in patients' homes, supporting mobility, strength, balance, and functional independence as a Home Health Physical Therapist (PT) based in Rockville, MD. This full-time, 13-week contract role offers a structured Monday through Friday schedule from 8:00 AM to 5:00 PM, with the opportunity to manage approximately 2532 points weekly. Engage in meaningful patient care, performing comprehensive evaluations, developing individualized treatment plans, and collaborating with interdisciplinary teams to achieve optimal outcomes.

Key Responsibilities:

• Conduct thorough physical therapy evaluations, including OASIS assessments for start of care, resumption, recertification, and discharge when applicable
• Design and implement personalized treatment plans focusing on functional mobility, gait training, pain management, and fall prevention
• Provide skilled interventions such as therapeutic exercises, neuromuscular re-education, transfer training, and patient/caregiver education on safety and home exercise programs
• Monitor patient progress, adjusting care plans as needed, and communicate changes to physicians and care teams
• Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB)
• Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides to ensure comprehensive care
• Participate in interdisciplinary meetings and adhere to agency policies and Medicare guidelines

Qualifications:

• Current Physical Therapist license in good standing for the state of practice
• Minimum one year of recent home health physical therapy experience, preferably field-based
• Required experience with OASIS assessments
• Familiarity with Home Care Home Base (HCHB) is preferred
• Reliable transportation, valid drivers license, and auto insurance essential
• Strong clinical judgment, communication, time-management, and organizational skills
• Ability to work independently in a field-based environment
