Seeking a dedicated, skilled physical therapist to deliver home health services for patients requiring personalized rehabilitation and support. In this position, you'll empower individuals to regain their independence by performing hands-on, high-quality therapy in the comfort of their own homes. The therapeutic specialty focuses on mobility, balance, safety, and functional goal achievement for diverse adult populations.
Schedule & Assignment
- Full-time contract, 13 weeks
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points per week (typically 57 daily visits, based on type)
- Field-based care with daily travel within a defined territory
Required Qualifications
- Valid and active Physical Therapist (PT) license in the state of practice
- Minimum 1 year of recent home health PT experience
- Proficiency with OASIS assessments and data collection
- Familiarity with Home Care Home Base (HCHB) preferred
- Dependable transportation, current drivers license, and active auto insurance
- Able to work autonomously, demonstrating sound clinical judgment
- Excellent interpersonal, documentation, and planning abilities
Primary Responsibilities
- Conduct detailed assessments and develop customized treatment plans
- Deliver skilled interventions: therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies
- Provide patient and caregiver education on exercises, equipment, and safety
- Accurately document all care in the Home Care Home Base system
- Coordinate with a multidisciplinary team, communicating patient progress and needs
- Attend interdisciplinary meetings, maintain compliance with agency and Medicare regulations
