Job Description
Make a difference each day by supporting patients in regaining independence and enhancing their quality of life as a Home Health Physical Therapist. In this contract role, youll deliver skilled therapy services to patients where they feel most comfortable their homes. Youll conduct comprehensive evaluations, create individualized care plans, and collaborate with a dedicated healthcare team, all while maintaining clinical excellence and compassion.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Qualifications:
- Active and unrestricted Physical Therapist license in the state of practice
- Minimum of 1 year of recent experience in home health physical therapy (field-based experience preferred)
- Proficiency in Home Care Home Base (HCHB) preferred; OASIS experience required
- Strong clinical assessment, functional evaluation, and decision-making skills
- Reliable transportation, valid drivers license, and auto insurance
- Organizational, time-management, and communication strengths
- Ability to work independently in a flexible, field-based environment
Key Responsibilities:
- Conduct thorough physical therapy evaluations, including OASIS assessments as necessary
- Develop and implement customized therapy plans focusing on mobility, gait, pain management, balance, and fall prevention
- Provide therapeutic interventions such as exercise, neuromuscular re-education, and transfer/gait training
- Monitor progress, updating treatments and communicating status changes with the care team
- Educate patients and caregivers on home exercise programs and safety strategies
- Maintain timely, accurate documentation in Home Care Home Base (HCHB) system
- Collaborate with nurses, therapists, and support staff for coordinated patient care
- Participate in case conferences and interdisciplinary meetings as needed
