The assignment is for 13 weeks, offering a great way to make a difference while enjoying flexibility and support.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Qualifications:

• Active Physical Therapist (PT) license in the applicable state and in good standing
• At least 1 year of recent experience as a Home Health Physical Therapist (field-based preferred)
• Working proficiency with Home Care Home Base (HCHB) preferred
• Experience with OASIS assessments required
• Demonstrated strength in clinical decision-making and functional assessment
• Valid drivers license, reliable transportation, and current auto insurance
• Excellent communication, organization, and patient education skills
• Comfort working independently and collaborating as part of an interdisciplinary healthcare team

Responsibilities:

• Conduct comprehensive therapy assessments, including OASIS for start of care and transitions
• Create and implement individualized plans of care targeting functional mobility, strength, and safety
• Deliver skilled interventions: therapeutic exercise, gait and transfer training, neuromuscular re-education, fall prevention, and pain management
• Monitor progress, adapt plans as indicated, and document care thoroughly using HCHB
• Educate patients and caregivers on home exercise programs, assistive devices, and safety strategies
• Collaborate closely with nurses, therapists, aides, and social workers to coordinate care
• Participate in interdisciplinary meetings, case conferences, and required continuing education
• Adhere to agency policies and Medicare/home health regulatory standards
