Deliver skilled, compassionate physical therapy to patients where it matters most: their homes. Physical therapists with experience in home health will thrive in this autonomous, patient-centered contract providing essential care and support to individuals seeking to regain function, independence, and quality of life.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Field-based visits: approximately 57 patient visits per day (2532 points/week)

Responsibilities:

• Complete thorough home health physical therapy evaluations, including OASIS assessments as required.
• Design and implement individualized care plans focused on mobility, safety, and patient-specific needs.
• Provide skilled interventions (therapeutic exercise, gait and transfer training, fall prevention, neuromuscular re-education).
• Continuously assess progress, adjust treatments, and communicate patient status to the interdisciplinary team.
• Educate patients and caregivers on exercises, safety, and assistive device use.
• Document all care, interventions, and progress promptly and accurately in Home Care Home Base (HCHB).
• Collaborate seamlessly with nurses, therapists, social workers, and home health aides.
• Adhere to agency policies and regulatory guidelines, including Medicare requirements.
• Attend team meetings and ongoing educational sessions as needed.
• Daily travel to patient homes within an assigned territory (mileage reimbursement provided).

Qualifications:

• Active Physical Therapist (PT) license in good standing for the state.
• At least 1 year of recent home health PT experience (field experience preferred).
• HCHB proficiency preferred; OASIS assessment experience required.
• Reliable transportation with valid drivers license and insurance.
• Self-motivated, organized, and able to work independently.
• Excellent clinical judgment, communication, and time management skills.
