This field-based position focuses on delivering personalized therapy services and empowering your clients to achieve their mobility and functional goals.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Qualifications:
- Active Physical Therapist (PT) license in good standing (state of practice)
- At least 1 year of recent Home Health Physical Therapy experience
- Experience with OASIS assessments required
- Proficiency in Home Care Home Base (HCHB) preferred
- Excellent clinical decision-making and assessment abilities
- Strong organizational, communication, and time management skills
- Reliable transportation, valid drivers license, and auto insurance
- Self-motivated with the ability to work independently
Responsibilities:
- Conduct thorough physical therapy evaluations, including start of care and resumption/recertification assessments
- Develop and implement tailored therapy plans focused on improving functional mobility, balance, gait, and pain management
- Deliver skilled treatment interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
- Monitor patient progress and adapt plans as needed based on clinical status
- Educate patients and caregivers on home exercise programs, safe mobility, and use of assistive devices
- Accurately document care using Home Care Home Base (HCHB) in compliance with agency and Medicare guidelines
- Work closely with an interdisciplinary team to ensure coordinated, high-quality care
- Participate in team meetings and continuing education as required
