Job Description
Provide skilled physical therapy services as a Home Health Physical Therapist Assistant (PTA) in patient residences, working under the supervision of a licensed Physical Therapist. This full-time, 13-week contract role requires delivering individualized care aimed at improving patients mobility, strength, balance, and functional independence.
Key Responsibilities:
- Implement skilled physical therapy treatments following established care plans.
- Assist patients with therapeutic exercises, gait and balance training, transfer techniques, and functional mobility.
- Monitor and document patient progress and response to treatments.
- Communicate changes in patient condition to supervising Physical Therapist and interdisciplinary team members.
- Educate patients and caregivers on safety strategies, assistive device use, fall prevention, and home exercise programs.
- Maintain timely and accurate documentation using Home Care Home Base (HCHB).
- Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and aides.
- Participate in case conferences and team meetings.
- Follow agency policies, regulatory standards, and Medicare home health guidelines.
Qualifications:
- Active Physical Therapist Assistant license in good standing in the state of practice.
- Minimum of 1 year recent Home Health PTA experience, preferably field-based.
- Proficiency with Home Care Home Base (HCHB) and knowledge of OASIS documentation preferred.
- Strong clinical observation, communication, organizational, and time management skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently and manage a flexible, patient-focused schedule.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM.
- Caseload approximately 57 patient visits daily, about 2532 points per week.
- Daily travel within assigned territory; mileage reimbursement provided.
