This contract opportunity supports adults seeking to restore or maintain function, maximize independence, and achieve meaningful rehabilitation outcomes in a comfortable, familiar setting.

Schedule & Details:

• Monday to Friday, 8:00 AM 5:00 PM
• 13-week contract; full-time hours
• Approximately 2532 points per week (about 57 patient visits daily)
• Daily travel to patient residences within the assigned territory

Requirements:

• Active, good-standing Physical Therapist license in the state of practice
• At least 1 year of recent home health physical therapy experience
• Proficient with OASIS assessments (Start of Care, Resumption, Recert, Discharge)
• Experience using Home Care Home Base (HCHB) preferred
• Strong clinical skills in assessment, functional mobility, balance, gait, and therapeutic modalities
• Reliable vehicle, valid drivers license, and current auto insurance
• Organized, self-motivated, and able to manage an autonomous, field-based schedule

Core Responsibilities:

• Complete comprehensive patient evaluations and individualized care plans focused on mobility, strength, balance, pain management, and safety
• Deliver evidence-based interventions such as therapeutic exercises, neuromuscular re-education, and gait training
• Educate patients and caregivers on exercises, safety, assistive devices, and home programs
• Maintain accurate, timely documentation in HCHB
• Collaborate with a multidisciplinary team to ensure coordinated, holistic care delivery
• Engage in interdisciplinary meetings and continuing education
