Deliver high-quality, patient-centered physical therapy services in a home health setting, making a profound difference in each patients independence and quality of life. This 13-week, full-time contract role requires a dedicated and compassionate Home Health Physical Therapist who thrives in an autonomous, field-based environment.

Schedule and Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: providing care in patient homes
• Caseload: Approximately 2532 points per week (typically 57 patient visits daily, depending on visit type)
• Daily travel within assigned territory; mileage reimbursement included

Key Responsibilities:

• Conduct comprehensive patient assessments using OASIS and develop personalized physical therapy care plans
• Deliver skilled interventions, including therapeutic exercise, neuromuscular re-education, gait training, and fall prevention techniques
• Educate patients and caregivers on home safety, use of assistive devices, and home exercise programs
• Track patient progress, adjust treatment plans as needed, and maintain clear communication with interdisciplinary team members
• Complete and maintain timely documentation in Home Care Home Base (HCHB)
• Consistently attend case conferences, collaborate across disciplines, and adhere to regulatory and agency standards

Qualifications:

• Current, active Physical Therapist (PT) license in good standing
• Minimum of 1 year recent Home Health Physical Therapy experience (field work strongly preferred)
• OASIS assessment experience required
• Proficiency with Home Care Home Base (HCHB) preferred
• Strong clinical assessment, communication, and organizational skills
• Ability to work independently and manage a flexible home health caseload
• Dependable transportation, valid drivers license, and auto insurance
