Job Description

Deliver exceptional patient care as a dedicated Home Health Physical Therapist, focusing on helping individuals regain strength, mobility, and independence in the comfort of their own homes. This contract position offers a rewarding balance of autonomy and collaboration with a supportive interdisciplinary healthcare team.

Schedule & Details:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Approximate caseload: 2532 points per week (57 patient visits per day)
  • Field-based role with daily travel within an assigned territory

What Youll Do:

  • Conduct thorough patient assessments and OASIS evaluations for start of care, resumption, recertification, and discharge, as needed
  • Develop and implement personalized plans targeting mobility, balance, strength, pain, and functional goals
  • Perform skilled therapy interventions such as therapeutic exercise, transfer and gait training, neuromuscular re-education, and fall prevention
  • Monitor, document, and communicate patient progress utilizing Home Care Home Base (HCHB)
  • Educate patients and caregivers on safe exercises, home safety, and the use of assistive equipment
  • Coordinate care with nurses, therapists, social workers, and home health aides
  • Attend case conferences and collaborate on treatment planning
  • Follow all regulatory requirements, agency policies, and Medicare guidelines

Qualifications:

  • Active PT license, in good standing, for the state of practice
  • At least 1 year of recent Home Health PT experience; field experience preferred
  • OASIS documentation experience required; proficiency with HCHB preferred
  • Solid clinical assessment, decision-making, and organizational abilities
  • Reliable transportation, valid drivers license, and up-to-date auto insurance
  • Comfort with independent, flexible, field-based scheduling
  • Excellent interpersonal and communication skills

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
