Provide expert physical therapy services as a Home Health Physical Therapist (PT) delivering patient-centered care in a field-based setting. This full-time, 13-week contract opportunity offers a dynamic caseload and the ability to make a positive impact on patients functional independence within their homes.

Responsibilities include conducting thorough physical therapy evaluationsincluding OASIS assessmentsdeveloping personalized treatment plans to improve mobility, strength, balance, and safety, and delivering skilled interventions such as therapeutic exercise, gait training, and fall prevention. You will monitor patient progress, update care plans accordingly, and collaborate closely with interdisciplinary healthcare professionals to ensure coordinated and effective care. Timely and accurate documentation using Home Care Home Base (HCHB) and adherence to Medicare home health guidelines are essential components of this role.

Qualifications and Desired Experience:

• Active Physical Therapist license in good standing for the state of practice
• Minimum one year of recent Home Health Physical Therapy experience; field experience strongly preferred
• Experience with OASIS assessments is required
• Proficiency in Home Care Home Base (HCHB) preferred
• Strong clinical decision-making, functional assessment, and communication skills
• Reliable transportation with a valid drivers license and auto insurance
• Ability to work independently and manage a flexible, field-based schedule

Schedule:

• Monday through Friday
• 8:00 AM to 5:00 PM
• Approximately 2532 points per week (57 patient visits daily depending on visit types)
