Bring your skills and compassionate care to patients homes as a Home Health Physical Therapist on a 13-week contract assignment. In this rewarding field-based role, you will assess, plan, and implement individualized therapy interventions to promote mobility, independence, and functional improvement. Youll work closely with interdisciplinary team members to ensure excellent patient outcomes, with the opportunity to manage your caseload and schedule, making a meaningful impact in home settings.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Physical Therapist (PT) license in the state of practice (in good standing)
- Minimum of 1 year of recent home health PT experience
- Proficiency with Home Care Home Base (HCHB) preferred
- Experience with OASIS assessments required
- Strong clinical assessment and decision-making skills
- Reliable transportation, valid drivers license, and auto insurance
- Effective communication, organizational, and time management abilities
- Ability to work independently and manage a flexible, field-based schedule
Primary Responsibilities:
- Complete comprehensive PT evaluations, including OASIS for Start of Care, Resumption, Recertification, and Discharge as needed
- Develop and execute personalized treatment plans focusing on mobility, safety, strength, and functional goals
- Deliver evidence-based interventions, such as therapeutic exercise, fall prevention, transfer training, and gait training
- Monitor patient progress, adjusting treatment plans and communicating changes to providers and care teams
- Educate patients and caregivers on safe mobility, exercise plans, and use of assistive equipment
- Maintain accurate, timely documentation through HCHB
- Participate in case conferences, interdisciplinary meetings, and professional development opportunities
- Ensure compliance with agency policies, regulatory standards, and Medicare guidelines

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
