Skilled Home Health Physical Therapist (PT) opportunity available in Pottstown, PA, offering a full-time, 13-week contract with a MondayFriday schedule from 8:00 AM to 5:00 PM. This role focuses on delivering patient-centered physical therapy care in home settings, enhancing patients mobility, strength, balance, and independence.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge when applicable
• Develop and implement tailored plans of care addressing functional mobility, gait training, pain management, and fall prevention
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, and transfer training
• Monitor patient progress, adjust treatments as needed, and communicate updates to physicians and the care team
• Educate patients and caregivers on exercises, safety techniques, assistive device usage, and home exercise programs
• Maintain accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides
• Adhere to agency policies, Medicare guidelines, and participate in interdisciplinary meetings and continuing education

Qualifications:

• Current and active Physical Therapist license in good standing for the state of practice
• Minimum one year of recent home health physical therapy experience; field experience preferred
• Proficiency with Home Care Home Base (HCHB) is preferred; OASIS experience required
• Strong clinical decision-making and functional assessment skills
• Reliable transportation, valid drivers license, and auto insurance to travel within assigned territory
• Excellent communication, time management, and organizational abilities
• Ability to work independently and manage a flexible, field-based schedule
