Job Description
An experienced and compassionate Home Health Physical Therapist (PT) is needed to provide skilled therapy services in patients homes. This role involves improving patients' mobility, strength, balance, and overall independence through individualized care plans. The ideal candidate should be organized, self-motivated, and comfortable delivering care independently while maintaining a focus on patient-centered outcomes.
Responsibilities:
- Conduct thorough physical therapy evaluations, including OASIS assessments when applicable
- Create and implement personalized treatment plans targeting mobility, strength, pain management, gait, and safety
- Provide therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies
- Monitor patient progress, modify plans as required, and communicate changes to physicians and team members
- Educate patients and caregivers on exercises, safety precautions, assistive device use, and home programs
- Maintain accurate electronic documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary team members such as nurses, occupational therapists, speech-language pathologists, and social workers
- Participate in case conferences, interdisciplinary meetings, and ongoing education
- Adhere to agency policies, regulatory standards, and Medicare home health guidelines
Qualifications:
- Active Physical Therapist license in good standing within the state of practice
- Minimum of one year recent home health physical therapy experience; field experience preferred
- OASIS assessment experience required; proficiency in HCHB preferred
- Strong clinical decision-making and functional assessment skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently in a flexible, field-based environment
- Excellent communication, time management, and organizational skills
Location: This opportunity is based in Pottstown, PA, an accessible community offering a balance of suburban comfort and proximity to metropolitan amenities.
Job Type: This is a full-time contract position lasting approximately 13 weeks, with a schedule of Monday through Friday, 8:00 AM to 5:00 PM. The caseload typically consists of 2532 points weekly, averaging 57 patient visits per day.
