Deliver high-quality, patient-centered care as a Home Health Physical Therapist Assistant, providing essential therapy services directly in patients' homes. This contract opportunity allows you to make a positive impact, assisting individuals to improve their mobility, strength, and independenceall while collaborating closely with licensed Physical Therapists and the broader care team.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Daily travel to assigned patient homes (mileage reimbursement provided)
• Caseload: Approximately 2532 points per week (57 visits per day, based on visit types)

Core Responsibilities:

• Deliver skilled physical therapy interventions in accordance with individualized treatment plans created by the supervising PT
• Support patients with therapeutic exercises, transfer and gait training, balance activities, and other mobility-focused interventions
• Educate patients and caregivers on safety, assistive devices, fall prevention, and the home exercise program
• Document patient progress and observations accurately using Home Care Home Base (HCHB)
• Communicate patient status, responses to treatment, and changes in condition to the supervising PT and interdisciplinary team
• Participate in team meetings and adhere to agency, regulatory, and Medicare standards

Desired Qualifications:

• Active Physical Therapist Assistant (PTA) license (in good standing) for the state of practice
• Minimum 1 year of recent home health PTA experience (field setting preferred)
• Familiarity with Home Care Home Base (HCHB) and OASIS documentation preferred
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment, time management, and communication abilities
• Comfortable working independently in a community setting

$1,609 - $1,702/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
