This contract role involves delivering tailored physical therapy services, guiding patients through their rehab journey, and fostering collaboration with a dedicated interdisciplinary team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active, unrestricted Physical Therapist license in Oregon
- At least 1 year of recent experience in Home Health Physical Therapy (field experience highly desired)
- Solid proficiency with Home Care Home Base (HCHB) preferred
- Experience conducting OASIS assessments required
- Strong clinical judgment and assessment skills
- Excellent organizational, time management, and interpersonal abilities
- Reliable transportation, valid drivers license, and auto insurance
Role Responsibilities:
- Complete comprehensive physical therapy evaluations, including OASIS assessments for start of care, recertification, and discharge as needed
- Develop and implement personalized plans of care to promote functional mobility, strength, balance, and safety
- Deliver skilled therapeutic interventions such as exercise, neuromuscular re-education, gait and transfer training, and fall prevention
- Monitor patient progress, update care plans, and communicate changes to physicians and the healthcare team
- Educate patients and caregivers on exercise routines, safety, and the use of assistive devices
- Maintain accurate, timely documentation within Home Care Home Base
- Collaborate in case conferences and interdisciplinary meetings
- Ensure adherence to regulatory standards and Medicare home health guidelines
