Deliver skilled, patient-centered care as a vital member of our home health team. This contract opportunity is designed for an experienced and compassionate Physical Therapist ready to make an impact by supporting patients within the comfort of their own homes. You will play a critical role in helping individuals regain and maintain functional independence while benefiting from a collaborative environment and supportive resources.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract assignment
• Caseload: Approximately 2532 points per week (typically 57 patient visits per day)

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments for varied care points.
• Develop and execute individualized therapeutic plans to improve patients mobility, strength, safety, and functional goals.
• Deliver interventions in therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
• Assess progress and adjust care plans to meet changing patient needs, collaborating closely with physicians and the healthcare team.
• Educate patients and caregivers on exercises, adaptive equipment, and safe mobility practices.
• Accurately document all visits and care provided using Home Care Home Base (HCHB) software.
• Participate in interdisciplinary case conferences and ongoing professional development.
• Ensure compliance with agency policies, Medicare guidelines, and all regulatory standards.

Required Qualifications:

• Current Physical Therapist license (in good standing) for the state of practice.
• Minimum one year of recent home health physical therapy experience.
• Solid familiarity with OASIS and field-based therapy; HCHB experience strongly preferred.
• Self-directed, highly organized, and effective in independent, field-based roles.
• Outstanding clinical judgment and communication skills.
• Valid drivers license, insured vehicle, and reliable transportation.

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
