Job Description
Deliver essential physical therapy services where it matters mostdirectly in patients homes. This full-time, 13-week contract position is designed for a Home Health Physical Therapist who thrives in a dynamic field-based environment, enjoys autonomy, and is passionate about improving patient independence.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive home health physical therapy evaluations, including required OASIS assessments.
- Develop, implement, and adjust individualized care and treatment plans targeting mobility, strength, balance, gait, and functional safety.
- Administer therapeutic exercises, gait and transfer training, fall prevention, and neuromuscular re-education.
- Monitor progress, modify interventions as necessary, and communicate updates to physicians and the healthcare team.
- Educate patients and caregivers on exercise routines, safety measures, assistive device use, and customized home exercise plans.
- Document all visits accurately and promptly using Home Care Home Base (HCHB).
- Collaborate with nurses, occupational therapists, speech-language pathologists, and social workers during case conferences and interdisciplinary meetings.
- Maintain compliance with agency policies, regulatory requirements, and Medicare home health standards.
Qualifications:
- Active and unrestricted Physical Therapist (PT) license in the state of practice.
- At least 1 year of recent experience in Home Health Physical Therapy, with strong field-based skills.
- Required proficiency with OASIS; experience with HCHB preferred.
- Reliable personal transportation, a valid drivers license, and current auto insurance.
- Excellent communication, clinical reasoning, and organizational abilities; able to work independently.
