Deliver high-quality, patient-centered care as a licensed Physical Therapist specializing in home health services. In this field-based role, youll empower clients to achieve greater independence and quality of life through skilled therapeutic interventionsright where they feel most comfortable. This contract opportunity is ideal for an organized, proactive clinician who thrives both independently and as a valued member of an interdisciplinary healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Duties:

• Complete comprehensive physical therapy evaluations and OASIS assessments as needed
• Develop, implement, and adjust individualized plans of care targeting functional mobility, strength, safety, balance, gait, and pain management
• Provide evidence-based therapy including therapeutic exercise, neuromuscular re-education, transfer and gait training, and fall prevention
• Educate patients and caregivers on exercises, safety protocols, home exercise programs, and use of assistive devices
• Document all care activities promptly and accurately in Home Care Home Base (HCHB)
• Attend case conferences, participate in interdisciplinary collaboration, and stay current with clinical best practices
• Comply with agency policies, regulatory guidelines, and Medicare standards
• Travel daily to patient homes within your assigned territory (mileage reimbursement offered)

Qualifications:

• Active Physical Therapist (PT) license in good standing in the state of practice
• At least 1 year of recent experience in home health physical therapy (field experience strongly preferred)
• Proficiency with OASIS and experience using Home Care Home Base (HCHB) is a plus
• Reliable transportation, valid drivers license, and auto insurance
• Excellent assessment, communication, and organizational skills
• Ability to work autonomously and manage a flexible schedule
