An excellent opportunity is available for an experienced and compassionate physical therapist to deliver skilled home health services, helping patients regain mobility, balance, and independence in the comfort of their own homes. This contract position offers a dynamic and rewarding environment for those confident in working autonomously while collaborating with a multidisciplinary healthcare team.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete comprehensive physical therapy evaluations, including OASIS documentation as required.
- Develop and implement individualized treatment plans targeting functional improvement, pain management, fall prevention, and patient safety.
- Administer therapeutic interventions such as exercise programs, neuromuscular re-education, gait and transfer training.
- Monitor patient progress, adjust care plans, and communicate changes to physicians and the healthcare team.
- Educate patients and caregivers on safety strategies, home exercise programs, and the use of assistive devices.
- Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
- Collaborate with nurses, occupational therapists, speech pathologists, social workers, and home health aides for coordinated care.
- Participate in case conferences and comply with regulatory and Medicare home health guidelines.
Qualifications:
- Active, unrestricted Physical Therapist (PT) license in the state
- At least 1 year of recent Home Health PT field experience
- Proficiency with HCHB preferred; OASIS assessment experience required
- Solid clinical decision-making, functional assessment, and organizational skills
- Dependable transportation, valid drivers license, and current auto insurance
- Ability to work independently and manage a flexible case load
