Schedule & Work Structure
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (typically 57 patient visits per day)
- Daily travel to patient homes (mileage reimbursement provided)
Core Responsibilities
- Perform comprehensive physical therapy assessments, including OASIS evaluations as needed
- Develop, implement, and adjust individualized plans of care to meet patient goals for mobility, balance, gait, and safety
- Deliver skilled interventions such as neuromuscular re-education, gait training, therapeutic exercise, and fall prevention strategies
- Educate patients and caregivers on exercises, use of assistive devices, and safety measures
- Document all visits accurately and promptly in Home Care Home Base (HCHB)
- Communicate effectively with physicians and the interdisciplinary team to ensure coordinated patient care
- Participate in meetings, continuing education, and maintain compliance with home health guidelines
Qualifications
- Active Physical Therapist license (in good standing)
- Minimum 1 year of recent home health PT experience (field experience preferred)
- OASIS assessment experience required
- Familiarity with Home Care Home Base (HCHB) preferred
- Reliable transportation, valid drivers license, and auto insurance
- Strong clinical, organizational, and communication skills
