Schedule & Work Structure

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Approximately 2532 points per week (typically 57 patient visits per day)
  • Daily travel to patient homes (mileage reimbursement provided)

Core Responsibilities

  • Perform comprehensive physical therapy assessments, including OASIS evaluations as needed
  • Develop, implement, and adjust individualized plans of care to meet patient goals for mobility, balance, gait, and safety
  • Deliver skilled interventions such as neuromuscular re-education, gait training, therapeutic exercise, and fall prevention strategies
  • Educate patients and caregivers on exercises, use of assistive devices, and safety measures
  • Document all visits accurately and promptly in Home Care Home Base (HCHB)
  • Communicate effectively with physicians and the interdisciplinary team to ensure coordinated patient care
  • Participate in meetings, continuing education, and maintain compliance with home health guidelines

Qualifications

  • Active Physical Therapist license (in good standing)
  • Minimum 1 year of recent home health PT experience (field experience preferred)
  • OASIS assessment experience required
  • Familiarity with Home Care Home Base (HCHB) preferred
  • Reliable transportation, valid drivers license, and auto insurance
  • Strong clinical, organizational, and communication skills
