Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active Physical Therapist license (in good standing) in the state of practice
• At least 1 year of recent experience in home health physical therapy, with field experience preferred
• Familiarity with Home Care Home Base (HCHB) electronic documentation is preferred
• Strong OASIS assessment experience is required
• Able to work autonomously, demonstrating strong clinical judgment and organizational skills
• Possession of reliable transportation, valid drivers license, and auto insurance
• Excellent communication skills and the ability to prioritize a flexible field-based caseload

Primary Responsibilities:

• Perform comprehensive patient assessments, including OASIS evaluations as needed
• Develop and implement individualized care plans targeting mobility, strength, balance, pain management, and safety
• Deliver interventions such as therapeutic exercise, transfer and gait training, neuromuscular re-education, and fall prevention
• Educate patients and caregivers on exercise regimens, safety techniques, and assistive equipment
• Maintain thorough and timely documentation utilizing HCHB
• Collaborate with interdisciplinary healthcare professionals to ensure coordinated, patient-centered care
• Regularly assess patient progress and adapt therapy plans as necessary; communicate updates to physicians and care teams
• Attend case conferences and pursue ongoing professional development
