This contract opportunity calls for an experienced and compassionate Home Health Physical Therapist to deliver skilled care that promotes mobility, strength, and independence.

Schedule & Assignment Details:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM
• Anticipated caseload: 2532 points per week (about 57 patient visits per day)
• Field-based: direct care in patients residences with daily travel within an assigned territory (mileage reimbursement provided)
• Electronic health records: Home Care Home Base (HCHB)
• Utilize OASIS for Start of Care, Recertification, and related assessments

Preferred Experience and Qualifications:

• Active Physical Therapist (PT) license, in good standing
• At least one year of recent home health PT experience, especially in field-based roles
• Proficiency with Home Care Home Base (HCHB) strongly preferred
• OASIS assessment competency required
• Solid clinical decision-making and functional assessment skills
• Reliable transportation, current drivers license, and auto insurance
• Ability to work autonomously and manage a flexible schedule
• Excellent communication, organizational, and time management abilities

Key Responsibilities:

• Conduct thorough physical therapy evaluations, including OASIS assessments when appropriate
• Create and implement custom plans of care focused on improving function, mobility, and safety
• Administer skilled interventions like therapeutic exercise, gait training, and fall prevention
• Continuously evaluate patient progress and adjust care plans accordingly
• Educate patients and caregivers in exercises, safety measures, adaptive equipment, and self-management
• Maintain precise and timely clinical documentation
• Collaborate with a multidisciplinary home health team to coordinate optimal patient care
• Attend case conferences and contribute to team-based patient management

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
