As a Home Health Physical Therapist, you will play a key role in fostering patient independence, safety, and well-being through personalized care and evidence-based interventions. This field-based contract position requires clinical autonomy, excellent organization, and a patient-centered mindset.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Qualifications:
- Active and unrestricted Physical Therapist (PT) license in the state of practice
- Minimum of one year of recent, hands-on home health physical therapy experience
- Solid command of OASIS assessments and electronic documentation (Home Care Home Base proficiency preferred)
- Reliable transportation, valid drivers license, and current auto insurance
- Strong communication, clinical decision-making, and time management abilities
- Confidence managing a flexible, independent field schedule
Key Responsibilities:
- Conduct thorough home-based PT evaluations, including OASIS assessments at required intervals
- Create and deliver individualized treatment plans targeting functional mobility, strength, balance, and safety
- Provide skilled therapies: exercise instruction, gait and transfer training, fall prevention, and patient/caregiver education
- Adjust interventions based on ongoing assessment and collaborate with physicians and interdisciplinary care teams
- Maintain accurate, timely documentation and participate in case conferences and ongoing education
- Uphold agency protocols and compliance with Medicare home health standards
