An exciting opportunity awaits for a skilled Home Health Physical Therapist to deliver individualized, patient-centered care within a supportive field-based setting. In this role, youll help patients regain mobility, strength, and independence by providing expert physical therapy services in the comfort of their homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract assignment

Key Qualifications:

• Active, unrestricted Physical Therapist (PT) license in the state of practice
• At least 1 year of recent Home Health PT experience, with field-based background preferred
• Proficiency in OASIS assessments required; experience with Home Care Home Base (HCHB) strongly preferred
• Excellent critical thinking and functional assessment abilities
• Strong organization, time management, and communication skills
• Valid drivers license, reliable transportation, and up-to-date auto insurance
• Ability to work independently while managing a flexible, mobile schedule

Primary Responsibilities:

• Conduct comprehensive in-home PT evaluations, including baseline and OASIS assessments
• Create individualized rehab plans targeting strength, functional mobility, safety, and pain reduction
• Deliver evidence-based interventions: therapeutic exercise, neuromuscular re-education, gait/transfer training, fall prevention
• Monitor and document progress via Home Care Home Base (HCHB); report changes to interdisciplinary team
• Educate patients and caregivers on home exercises, mobility devices, and personalized safety strategies
• Collaborate with nurses, occupational/speech therapists, social workers, and home health aides to ensure coordinated care
• Participate in relevant team meetings and remain current with industry best-practices
