Deliver impactful patient care as a Home Health Physical Therapist (PT) by providing skilled therapy services directly in patients homes. In this role, youll empower individuals to regain independence through functional mobility improvements, exercise programs, and patient-centered care. Schedule & Assignment Details:

• Monday to Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Daily travel required within a designated area (mileage reimbursement included)
• Typical caseload: 2532 points per week (about 57 visits daily)
• Utilize Home Care Home Base (HCHB) for electronic documentation
• OASIS assessments required for Start of Care, Resumption, Recertification, and Discharge

Desired Skills & Qualifications:

• Active, unrestricted Physical Therapist (PT) license in the state
• At least one year of recent experience in Home Health Physical Therapy
• Demonstrated ability with OASIS and electronic documentation (HCHB experience preferred)
• Solid functional assessment, clinical decision-making, and organizational skills
• Strong communication, self-motivation, and ability to work independently
• Reliable transportation, valid drivers license, and auto insurance

Key Responsibilities:

• Perform comprehensive PT evaluations and OASIS assessments as needed
• Develop and implement customized plans of care with a focus on mobility, strength, balance, and safety
• Deliver skilled interventions including therapeutic exercise and gait training
• Educate patients and caregivers on exercises, assistive devices, and home safety
• Track progress, adjust plans, and communicate effectively with the broader healthcare team
• Attend case conferences and interdisciplinary meetings for coordinated care
• Uphold regulatory requirements and agency policies throughout care delivery
