Job Description

Take your clinical expertise into the homes of patients as a dedicated home health physical therapist. This full-time, 13-week contract role is designed for compassionate PTs eager to promote independence, restore mobility, and directly influence patient outcomes through one-on-one care.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key responsibilities:

  • Perform comprehensive physical therapy evaluations, including OASIS assessments.
  • Develop and implement effective, individualized therapy plans aimed at improving mobility, balance, strength, safety, and pain management.
  • Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
  • Monitor patient progress, adjust treatment strategies as needed, and communicate with physicians and care teams.
  • Educate patients and caregivers in home exercise programs, use of assistive devices, and safety techniques.
  • Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
  • Collaborate with nurses, occupational and speech therapists, medical social workers, and aides to ensure comprehensive, coordinated care.
  • Participate in interdisciplinary meetings, case conferences, and ongoing professional development.
  • Remain compliant with agency policies, Medicare home health guidelines, and regulatory standards.

Qualifications:

  • Active and in-good-standing Physical Therapist (PT) license in the state of practice
  • At least 1 year of recent home health PT experience (field-based experience strongly preferred)
  • Required experience completing OASIS assessments
  • Strong clinical reasoning, organization, and assessment abilities
  • Proficient with Home Care Home Base (HCHB) software strongly preferred
  • Valid drivers license, reliable transportation, and auto insurance
  • Able to manage a flexible schedule and work independently
