Job Description

Bring your passion for hands-on, patient-focused care to an essential home health role supporting individuals in their journey toward greater independence. As a Home Health Physical Therapist, you will deliver personalized, high-quality therapy to patients in their homes, empowering them to regain mobility, strength, and daily function.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract
  • Approximately 2532 points per week (typically 57 patient visits per day)
  • Daily travel within assigned territory (mileage reimbursement available)

Desired qualifications:

  • Active Physical Therapist license (PT), in good standing
  • Minimum 1 year of recent Home Health Physical Therapy experience
  • Experience with OASIS assessments
  • Proficiency in Home Care Home Base (HCHB) preferred
  • Excellent communication and organizational skills
  • Reliable transportation, valid drivers license, and auto insurance
  • Ability to work independently in a field-based setting

Key responsibilities:

  • Conduct comprehensive physical therapy evaluations and OASIS assessments as required
  • Develop and implement individualized plans of care, focusing on functional mobility, strength, and safety
  • Provide skilled interventions, including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies
  • Educate patients and caregivers on treatment goals, exercises, safety, and use of assistive devices
  • Modify treatment plans based on patient progress and communicate changes to the healthcare team
  • Maintain timely and accurate documentation in HCHB
  • Collaborate with interdisciplinary team members to ensure continuous, coordinated care
  • Participate in case conferences and continuing education as needed
  • Adhere to agency policies and Medicare home health guidelines
