Job Description
Deliver essential Home Health Physical Therapy services while supporting patients' independence and mobility in a field-based role. This contract position offers the satisfaction of one-on-one care in patients homes, with a focus on function, safety, and interdisciplinary collaboration.
Schedule/Hours:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Qualifications:
- Active Physical Therapist (PT) license in good standing for the state
- At least 1 year of recent Home Health Physical Therapy experience (field experience strongly preferred)
- Proficiency with Home Care Home Base (HCHB) preferred
- OASIS assessment experience required
- Strong clinical decision-making and assessment capabilities
- Reliable transportation, valid drivers license, and current auto insurance
- Independent work skills and excellent communication and organizational strengths
Responsibilities:
- Conduct comprehensive in-home PT evaluations, including OASIS documentation when applicable
- Develop individual treatment plans aimed at improving functional mobility, strength, balance, and overall independence
- Provide skilled physical therapy interventions (therapeutic exercise, gait and transfer training, neuromuscular re-education, pain management, fall prevention)
- Educate patients and caregivers on home exercise routines, safety strategies, and equipment use
- Adapt plans of care as needed, monitor patient progress, and update care team and physician when appropriate
- Document all care and progress in Home Care Home Base (HCHB)
- Collaborate with a diverse healthcare team to ensure coordinated, high-quality care
- Attend interdisciplinary meetings, case conferences, and maintain regulatory compliance
