Experienced Home Health Physical Therapist (PT) Opportunity Ocala, FL
A rewarding opportunity is available for a skilled and compassionate Home Health Physical Therapist to deliver patient-centered care within the comfort of patients' homes. This role focuses on improving mobility, strength, balance, and overall functional independence through personalized therapeutic interventions. Contract Details:
- Full-time, 13-week contract
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points weekly (57 patient visits/day, variable)
- Setting: Patient homes within an assigned territory with mileage reimbursement
- Documentation: Home Care Home Base (HCHB) electronic system
- Assessment tool: OASIS (Start of Care, Recertification, Resumption, Discharge)
Primary Responsibilities:
- Conduct comprehensive physical therapy evaluations and utilize OASIS assessments as required
- Develop and implement customized care plans targeting strength, balance, gait, pain management, and safety
- Provide skilled interventions including therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention
- Monitor patient progress, adjust treatment plans accordingly, and communicate updates with physicians and care teams
- Educate patients and caregivers on exercises, safety, assistive device use, and home exercise programs
- Maintain accurate, timely documentation in HCHB software
- Collaborate closely with interdisciplinary teams such as nursing, occupational and speech therapy, social work, and home health aides
- Comply with agency policies and Medicare home health regulations
Qualifications:
- Current Physical Therapist license in-state (good standing)
- Minimum 1 year recent Home Health Physical Therapy experience; field experience preferred
- Proficiency with HCHB preferred; OASIS experience required
- Strong clinical decision-making and functional assessment capabilities
- Reliable vehicle, valid drivers license, and insurance
- Ability to manage flexible, independent field schedule
- Excellent communication, time management, and organizational skills
