Job Description

An experienced Home Health Physical Therapist (PT) is needed to provide skilled therapy services in patient homes within the Oakland, OR area. This full-time, 13-week contract role involves evaluating patients, developing personalized treatment plans, and collaborating with healthcare professionals to promote mobility, strength, balance, and overall functional independence.

Key Responsibilities: Perform comprehensive PT evaluations and OASIS assessments (Start of Care, Resumption, Recertification, Discharge) Develop and implement individualized plans of care focusing on gait training, therapeutic exercise, neuromuscular re-education, pain management, and fall prevention Monitor patient progress, adjust treatments, and communicate changes to physicians and care team members Educate patients and caregivers on home exercises, safety, and assistive device use Accurately document patient records using Home Care Home Base (HCHB) Collaborate with interdisciplinary team members for coordinated patient care Participate in case conferences, meetings, and continuing education Comply with agency policies and Medicare home health regulations Qualifications: Active Physical Therapist license in good standing within the state Minimum of 1 year recent home health physical therapy experience; field experience preferred Proficiency with HCHB documentation system and required OASIS assessments Strong clinical decision-making and functional assessment skills Reliable transportation, valid drivers license, and insurance Ability to work independently with excellent organizational and communication skills Schedule/Hours: Monday Friday, 8:00 AM 5:00 PM Approximately 2532 productivity points weekly (57 patient visits per day) Location and Work Environment: This role is based in Oakland, OR, providing services in patient homes throughout the community.
