A rewarding opportunity is available for a skilled and compassionate Home Health Physical Therapist to deliver exceptional care within patients homes. This contract position focuses on fostering mobility, strength, and independence through individualized therapy plans and patient-centered care.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based, providing care in patient homes
• Approximately 2532 points per week (typically 57 visits per day, depending on type)
• Daily travel within an assigned territory (mileage reimbursement offered)

Core Responsibilities:

• Conduct thorough physical therapy evaluationsincluding OASIS assessments where requiredto design and monitor patient-specific treatment plans.
• Deliver therapeutic interventions such as exercise, gait and transfer training, neuromuscular re-education, and fall prevention techniques.
• Evaluate patient progress, modify interventions as needed, and collaborate closely with physicians and interdisciplinary care teams.
• Educate patients and caregivers on rehab exercises, home safety, equipment use, and self-management strategies.
• Complete timely and accurate documentation using the Home Care Home Base (HCHB) electronic system.
• Participate in team meetings and ongoing professional development activities.
• Remain compliant with agency standards, Medicare guidelines, and home health regulations.

Required Qualifications:

• Current, active Physical Therapist (PT) license in good standing
• Minimum one year of recent home health physical therapy experience, with strong functional assessment skills
• OASIS assessment proficiency and familiarity with Home Care Home Base (HCHB) preferred
• Self-motivated, independent, and organized, with excellent communication skills
• Reliable transportation, valid drivers license, and current auto insurance
